Important Information about Health and Dental Insurance

*  In order to be guaranteed insurance, a pastor must apply within 90 days of becoming employed in a UCC church and remain in the plan.
* Those who do not apply or remaining the plan must complete a statement of health.
These people are not guaranteed insurance.
* Lay employees who work 20 hours a week or more may apply for health and dental coverage. These employees have 90 days from their start date to be guaranteed insurance. It makes no difference if the church did not offer insurance at the time of their employment.

Annual Meeting Registration
* Registration materials are received approximately 2 months before event.
* Checks should accompany completed registration and include nothing except annual meeting registration.
* Please remit to be received by the due date on the registration.
