Important information about health and dental insurance
OCWM (Our Church's Wider Mission)
 | Goals are set by the Conference office in the fall for the following year. |
 | Blank remittance forms arrive from the Conference office in January. |
 | Return a completed remittance form with payment - special offerings listed on the
bottom. |
 | Prefer payment monthly. |
 | Will accept quarterly, semiannual and annual payments. |
 | Other offerings (list below) not specifically named on the remittance form:
Offerings from installations and ordinations
Disaster relief (floods, earthquakes, fires, etc.)
Blankets, Heifer funds, Church World Service

Insurance (Health and Dental)
Plan: Pension Boards (Plan A)
 | Invoices will arrive quarterly. (January, March, June and September) |
 | Please return a copy of the invoice with check |
 | All churches owe the per capita even when the church has no minister or the minister is
covered by another plan. Per capita is the benevolence portion of our insurance
program which allows all churches and retired pastors to be covered with health insurance. |
 | Health and Dental - Amounts are set by the Conference office, approved by
the Insurance committee and Board of Directors. |
 | Please only include insurance on your check. |

Annual Meeting Registration
 | Registration materials are received approximately 2 months before event. |
 | Checks should accompany completed registration and include nothing except annual
meeting registration. |
 | Please remit to be received by the due date on the registration. |

Association Dues

These are payments made annually at the first of the year or quarterly
throughout the year.
Your Association will send you a notice in January with the amount to
pay in the current year. |

These are payments made annually at the first of the year or quarterly
throughout the year.
Your Association will send you a notice in September with the amount to
pay in the next year. |

In November or December, your Association sends a letter addressed to
the pastor. You must fill in the membership figure and multiply by the per
capita
amount to figure your dues for the following year. You should return a copy of this
form with your payment to the Association Office. You may pay in one installment,
quarterly, or monthly. |

In December, your Association sends you a letter with the amount you
owe for the upcoming year. This amount is figured per member using the current
yearbook numbers. You may pay either the entire amount in January or make payments
throughout the year. The Association will send reminder notices in the fall for
outstanding balances which are due by the end of December. |

Life and Disability Group Life
 | Invoiced quarterly with the Minister's Pension |
 | If you receive an invoice from the Pensions Boards which includes health and dental,
please pay only the required employer dues, group life and life/disability to the Pension
Boards. |
 | Send a copy of the invoice to the Conference for payment of health and dental insurance. |

Important Information about Health and
Dental Insurance
 | In order to be guaranteed insurance, a pastor must apply within 90 days of becoming
employed in a UCC church and remain in the plan. |
 | Those who do not apply or remaining the plan must complete a statement of health. |
 | These people are not guaranteed insurance. |
 | Lay employees who work 20 hours a week or more may apply for health and dental coverage.
These employees have 90 days from their start date to be guaranteed insurance.
It makes no difference if the church did not offer insurance at the time of their
employment. |
